Assumption of Risk and Waiver of Liability Relating to COVID-19

NSW Friendship Circle Inc* has put in place preventative measures to reduce the spread of COVID-19; however, we cannot guarantee that the Participant will not become infected with COVID-19. Further, attending NSW Friendship Circle Inc’s programs could increase the Participant’s risk of exposure to COVID-19.

By accepting this agreement:

1. I acknowledge the contagious nature of COVID-19;
2. I voluntarily assume the risk that the Participant may be exposed to or infected by COVID-19 by attending NSW Friendship Circle Inc programs and that such exposure or infection may result in personal injury, illness, permanent disability, and death;
3. I understand that the risk of becoming exposed to or infected by COVID-19 at NSW Friendship Circle Inc programs may result from the actions, omissions, or negligence of those associated with NSW Friendship Circle Inc, including, but not limited to its staff, volunteers, program participants, and their families and members of the public;
4. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for and hold NSW Friendship Circle Inc harmless for, any injury to the Participant, illness, damage, loss, claim, liability, or expense, of any kind, that I or the Participant may experience or incur in connection to the Participant’s attendance at NSW Friendship Circle Inc programs.
